Prepaid services, services that have been rendered, and services that have been specifically prepared or ordered for a patient are non-refundable. ## Hormone Optimization Memberships Hormone Optimization treatment is provided through a monthly or prepaid membership program. The following cancellation and refund policies apply to Hormone Optimization memberships only. ### Monthly Memberships Monthly memberships may be canceled by providing at least 30 days’ notice prior to the next scheduled billing date. Any outstanding balance must be paid before cancellation is finalized. Membership fees already charged are non-refundable. Because hormone therapy is medically managed, patients are encouraged to discuss discontinuing treatment with our clinical team before canceling their membership or stopping their medication. ### Prepaid Memberships Prepaid membership plans are offered at a discounted rate in exchange for a longer-term commitment.
