THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND 

HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

  1. How We May Use and Disclose Health Information 

Vital Society may use and disclose your protected health information (PHI) for treatment, payment, and healthcare operations. For example, we may use your information to coordinate your care with another provider, to bill for services, or to evaluate the quality of care we deliver. 

We may also disclose information when required or permitted by law, including public health activities, abuse or neglect reporting, health oversight audits, law enforcement requests, coroners and medical examiners, workers’ compensation, or national security requirements

  1. Uses and Disclosures Requiring Authorization 

Other uses and disclosures of your PHI will be made only with your written authorization. You may revoke your authorization at any time in writing, except to the extent that action has already been taken

  1. Your Rights Regarding Health Information 

You have several rights regarding your health information. You have the right to inspect and obtain a copy of your medical record and to request an amendment if you believe the information is incorrect or incomplete. You may request restrictions on certain uses or disclosures of your information and ask that we communicate with you in a way that better protects your privacy, such as by using a different phone number or mailing address. You also have the right to request an accounting of disclosures of your protected health information, to receive a paper copy of this notice at any time, and to file a complaint if you believe your privacy rights have been violated.

Vital Society is required by law to protect the privacy of your health information and to provide this notice explaining our privacy practices. We will follow the terms of the notice currently in effect and will post an updated version in our office and on our website if any changes occur

  1. Compliance with Texas Law 

In addition to federal privacy protections, Texas law provides additional safeguards for your health information. Vital Society complies with all applicable federal and Texas privacy laws regarding the protection of your medical information
